A former caretaker at a Grand Junction assisted-living facility has pleaded guilty to neglect in the 2021 death of an 86-year-old resident, according to a Friday news release from the
State comptroller examined New Jersey’s lowest-rated nursing homes participating in Medicaid program. 15 out of 368 were identified as worst long-term care facilities, ranked one-star by Center for Medicare and Medicaid Services CMS due to staffing, health inspections, etc.
The suit says Canyon Creek’s owner- Olympia, Washington-based Koelsch Communities- knowingly kept the facility understaffed to save money and increase revenue.
