6月25日，甘肃省政府网站公布造成21死8伤的“白银马拉松事故”调查报告全文。报告全文约1.9万字，内容包括赛事基本情况，事件发生经过等6个方面。
其中在事发经过中，涉及检查点（check point）人员配备、天气变化与体感温度关联、从开赛到出事再到救援的整个时间轴。
报告显示，5月22日9时比赛开始，至11时50分有跑手通过GPS定位设备发出求救信息，但未得到回应。中午12时17分左右，赛事营运及工作微信群陆续收到求救信息，并各自展开救援工作，但没有叫停比赛。至下午2时10分左右，大批跑手退赛，赛事被迫中止。
当日死伤主要发生在2号检查点至3号检查点间的路段，报告显示在海拔1347米的2号检查点，配备义工7人、医护3人、救护车1辆，但海拔2230米的3号打卡点，仅配备义工2人。
报告称，“赛道补给点设置不合理，在最难最险的高海拔赛段（2230米）未设置医疗救助和补给点；未采取加强和改善通讯条件的措施，导致最危险时刻通讯联络不畅”；赛事承办执行和营运单位未按规定制定专项应急预案和保安措施，应急救援力量准备严重不足等问题，亦是造成参赛者大量伤亡的直接因素。
报告还称，白银市委市政府、景泰县委县政府等部门对赛事组织管理不规范；安全监管措施不落实；救援力量准备不到位；安全保障条件不充份，是造成事故的间接原因。景泰县委书记李作璧被指应对事件负主要领导责任。
6月9日，李作璧家中跳楼自杀。6月11日，甘肃当局公布27名公职人员被问责，多数被警告。
网民对官方处理表示不满。认为21条人命换来的只是处分，记过，警告，是官场的走形式。
