Complete Revascularization Analysis of ISCHEMIA Opens 'Pandora's Box'

Patients with stable coronary artery disease who received “complete” revascularization in the ISCHEMIA trial appear to have derived more clinical benefit with the invasive strategy than medical therapy over follow-up, a finding at odds with what was observed for the overall trial. The implication, say investigators, is that revascularization might offer clinical advantages over conservative medical therapy if all anatomically relevant lesions are treated.
This analysis, which focused only on the patients who had complete revascularization defined anatomically, showed that the primary composite endpoint occurred in 11.9% of patients treated invasively as opposed to 15.4% of patients who were treated medically. ....

Emergent CABG in Acute MI: Survival Better Despite Increasingly Risky Patients

Emergent CABG surgery has seen waning use over the past few decades among patients hospitalized for acute MI in the United States, a trend not likely to come as a surprise to cardiologists watching PCI’s ascendance. But nationwide data also show that even as the patients targeted for CABG therapy have grown sicker and higher-risk, their in-hospital mortality rates have decreased.
The data, encompassing more than 11 million acute MI admissions from 2000 to 2017, were released today during the virtual American College of Cardiology (ACC) 2021 Scientific Session and simultaneously published online in the
Journal of the American Heart Association.

Last spring, during the height of the COVID-19 pandemic, James Harvey, MD (WellSpan Health, York, PA), a specialist in valvular heart disease, was the interventional cardiologist covering his institution’s cath lab when a patient with critical limb ischemia was admitted with an acute vessel closure severe enough that the patient was dangerously close to losing a leg.
“I had trained in this and I had done it, but it really hadn t been my forte in a while,” Harvey recalled to TCTMD. “I needed some guidance.”
