MIAMI, FL A debate at TVT 2021 last week was supposed to be about whether guideline-directed medical therapy should be tried at maximally tolerated doses before patients undergo transcatheter edge-to-edge repair (TEER) for functional mitral regurgitation (MR), but it ended up focusing on the difficulty of getting patients on foundational heart failure therapies in the first place.
JoAnn Lindenfeld, MD (Vanderbilt Heart and Vascular Institute, Nashville, TN), led off by arguing that drug therapy should be maxed out first, but her ostensible opponent didn’t disagree.

CLASP 2-Year Data Promising for Pascal Transcatheter Mitral Repair

Preliminary 2-year results from the single-arm CLASP study show that mitral valve repair using the Pascal transcatheter system is associated with high rates of survival and freedom from rehospitalization as well as a durable reduction in mitral regurgitation (MR).
According to Molly Szerlip, MD (Baylor Scott & White Health, Plano, TX), who virtually presented the study as a late breaker at EuroPCR 2021, the data look “extremely good,” at least for the numbers of patients followed so far, in both functional and degenerative MR.
“This reiterates that this is an excellent device and one that needs to be out on the market,” she told TCTMD. “And as the long-term data come out, there s nothing that would make us think that [this device] is not going to continue to have excellent long-term outcomes.” ....

Outcomes Worse for STEMI Patients Without Obvious Risk Factors

STEMI commonly occurs in the absence of four common CV risk factors hypertension, diabetes, hypercholesterolemia, and current smoking and when it does, outcomes can be worse than those seen in the larger STEMI population, data from the SWEDEHEART registry indicate.
All-cause mortality at 30 days was significantly higher in patients without one of these standard modifiable CV risk factors (11.3% vs 7.9%; HR 1.47; 95% CI 1.37-1.57), dubbed SMuRFs by Gemma Figtree, DPhil (Royal North Shore Hospital, Sydney, Australia), and colleagues.
The disparity was exacerbated in women SMuRF-less women had the highest rate of 30-day mortality (17.6%), followed by women with traditional risk factors (11.1%). Mortality risks were lower in men, with a smaller difference between those who didn’t versus did have SMuRFs (9.3% vs 6.1%). ....
