And from the secretary of Homeland Security himself jay johnson testify in our lack of ability to gather information. Theres the f. B. I. They have testified theres no system on the ground to collect information. Theres no f. B. I. Office in syria. Theres no one we can count on and the secretary himself said that theres a lot that we dont know about the people that we are letting in. Thats National Security risk. And i think it boils down just that. Allowing Syrian Refugees into the United States when isis has claimed that no blood is better than american plood and that they will taste it soon. I think its irresponsible for us to turn our heads and not need warnings of National Security experts who cannot collect any background information. So we have a lot of talk about the rigorous background checks that are done in the Refugee Program, and that may be true, but that doesnt mean that thats in place in syria. And thats what we are talking about. We are talking about taking refugees from syria when we know we have no way to interview them or records to look at and what we do know is that isis wants to come here and kill us. Host he says a Homeland Committee goes to syria and interviews those who want to come here and the process takes 1824 months and the people coming here are prioritized women and children and victims of terror and also those over age 60. Only 2 of those who come here are males of socalled fighting age. Golic so were counting on them to tell you the truth . Or are we relying on assads government who we are trying to overthrow to hand over to us the records of individuals so nobody comes to us to harm us . This is ludacris. We have no system or way to depathter true information of what this true person is or their motivation. So that argument is not valid. From 38 countries you can come to the United States without a visa or personal interviews for 90 days. We have open borders, 1120 immigrants here who we know nothing about. Nearly half the people in the country here illegally come here on an overstayed visa and we cant find them. We have holes in our system. And if we continue to roll the dice and now accepting refugees from syria without any proper background checks, this is in total contrast to the 9 11 Commission Report. I carry that with me most times to my Homeland Security hearings. That report, this was a commission put together after september 11th to come back to congress and make recommendations on what we should do so this never happens again. They liked it so much that Congress Passed it and the president signed it and put it into law and we have not looked at it since. One, they want to get here and be here until they carry out their mission. Why would we assist them when we know we are at war with isis who wants to use this program sand willing to use this program to do us harm . Host another raid in paris. Joining uslando kurd from the cspan bus to talk about the treatment of anxiety and postTraumatic Stress disorders, especially among the military and veteran population is dr. Deborah by doubt. The director of the universitys postTraumatic Stress disorder clinic. Thank you very much for being with us this morning. Tell us about the universitys clinic and why you have this specifically at the university. Morning, im happy to be here today. We had a stress disorder clinic at the university of Central Florida, because it is very important average to veterans. The field of in anxiety for 30 years. And, when i moved here, we wanted to open a clinic that would treat both Anxiety Disorders and postTraumatic Stress disorder. Privileged that we work with armys military operation search program. They have given us a very generous Grant Funding to set up the clinic and conduct research on effective treatments for combat veterans. You have received a 5. 3 million deal. The only threeis weeks Treatment Program in the nation. It has been dubbed as comprehensive. Explain what it is an wide is the only one in this nation. Im happy to. It is a program that combines exposure therapy. Exposure therapy is the most scientifically researched treatment for trauma and postTraumatic Stress disorder. Asks people to put themselves back in the situation that created the stress. Example is how you get over your fear of a dog, you probably say well out have to be around a dog. We have to do the same thing with postTraumatic Stress disorder. What we know is that we cannot always recreate the horrific events of the war. We have added a Virtual Reality. Where, we can have people experience the sights, sounds, and the smells of war in order to make the exposure therapy more effective. Now, exposure Therapy Works very well for the fear aspects of postTraumatic Stress disorder. Bus, we know from our veterans is that they also experience other emotions like anger, sometimes depression, and sometimes a social isolation. It is something called moral injury. To the exposure therapy which a added people do, we have a good treatment component that specifically addresses these issues. Is reason for this program that a first of all, exposure Therapy Works better when you do the sessions and a close fashion. Second thing, we want to have treatment that works not only for veterans, but would work for active duty as well who cannot be out of activeduty for 12 or 18 weeks. So, we developed the program where people get to treatment sessions a day. Exposure therapy in the morning. Group therapy in the afternoon. We are privileged that we can offer free accommodations to people who qualify for the research program. All they have to do is get themselves tour land of, with the generous funding we have received, we can take care of the rest. Including for treatment. Host what do you see when youre exposing these veterans to what they saw combat. What is the reaction that you see . Well, initially, people will have an anxious or fair reaction. A lot of them feel they will lose control. People do not lose control. They do become anxious, but we do not leave people alone. We stay with them during the session until the anxiety comes back down. We do this over a number of days. What we find is that each day, their initial anxiety response becomes less and less. It does not take as long for them to say ok, im not feeling anxiety anymore. I want to make it clear that were not trying to raise peoples memories or do anything like that. The goal of treatment is for people to be able to take back their life. If i can give you an example, people tell us they cannot drive on interstate highways. Here, they say they cannot drive on i4 because theyre looking at things on the side of the road for iuds. They will say that they know there is no ied in orlando. Trash on the some side of the road come i get anxious, i get scared. I may pull off the road. I may drive into a ditch because im so upset and anxious about what looks like an ied. Our goal is to get people to have control of their life. Not to forget what happened to them, that would be impossible. We want to get them to feel comfortable again. The combination of our individual treatment with Virtual Reality, and adding in the group component, also the study is not quite finished. Right now, more than 64 of the people no longer have postTraumatic Stress disorder. And, more importantly, when you follow them up, six months later, the real upgrade is less than 10 . Were very excited about the results we have gotten so far. As a scientist, we do have more work to do. But, we are getting very effective results. Were talking with dr. Deborah by doubt. She is the stress disorder clinic. Were at the university of Central Florida in the Sunshine State tour. Campaign 2016 buses there. It is part of for University Stops that were making this week talking to different experts. Today, we talking about the issues of postTraumatic Stress disorder and anxiety. I will cut you a little bit about the university. It is located in orlando. It has 13 additional campuses. Founded in 1963. They have an enrollment of 60,000 graduate students. Undergraduates in state have around 6000, outofstate is closer to 21,000. They have an endowment over 155 million. Your doingck to what there. And why youre doing it. Let me ask, why is it that the university of Central Florida is doing this work and at the Veterans Affairs department . Think the department is doing some work that they can do. The advantage of being at a place Like University of central we are notthat constrained by the regulations that the Veterans AffairsDepartment Might have. So, for example, we can offer. Reatment in this fashion where, in some cases, the number of people seeking services might not allow a person to be seen every day for three days in a row. So, we have some flexibility in what it is we can do. We do not see ourselves as doing something instead of the Veterans Administration. We see is just another alternative. Perhaps for people who have not had successful treatment at the Veterans Administration. Or for people who cannot get treated there. For example, in our threeweek of our participants are activeduty personnel. So, theyre not yet veterans. Their supposedly veterans of the conflict. Offere are able to activeduty personnel that service as well. Were doing things a little bit differently. We think we are these things in a way that will be helpful for people who have failed out of more traditional outpatient program. Host how quickly can you see these people who need help . Guest well, we can see everyone who calls us. We do an initial telephone screening, because our research anding, the treatment is not experimental treatment. There are treatments that have been used. It is the combination that is part of the search program. We can have a phone appointment with somebody within a month. What we do is people come here as a cohort. That is a fancy word for group. We have six or seven people who come at the beginning of a month. They move through the program across the three weeks. We take a week to get everybody settled. Write all of our notes. Next is the next month. Depending on when people come, they might be able to start treatment in two weeks or one month. If they are local, and they want our outpatient Treatment Program. They can have the first appointment within one week. Host talk about what is postTraumatic Stress disorder. Is it mostly people, or does it have to be attributed to combat. We talk about two kinds of postTraumatic Stress. With iraq combat related, and we talk about civilian related. Quite frankly, trauma is all around us. All of us knows somebody who has been impacted by trauma. We conduct about people who are crime victims. Of thetalk about all people who do with struggles in their daily life. We can talk about natural disasters. About people who are victims of Sexual Assault or abuse. Many different kinds of things. Motor vehicle accidents are a traumatic event. So, we are talking about 90 of people in the United States where it some point in their life, they experience it or mac event. No, for most of us, we end up with a time of resilience our recovery. So, we might have symptoms for a few days. Then, after an amount of time we feel better. But, there are some people who have overcome postTraumatic Stress. Which means that they may have symptoms of hyper digital its. Always be on guard. Never been able to relax. Talk about how i need to keep my eyes up for danger of the time. Veterans will say i cannot sit with my back to adore, because i cannot see who might be coming through the door. We talk about problems with nightmares. We talk about problems with difficulty sleeping. Just being able to get to sleep and wake up. People with postTraumatic Stress stress disorder, whether this combat or civilian, talk about something they call the sense of it for short. What they really mean is that they feel that they have changed their life so much that their life is going to be shorter. They will not live as long as other people. It is a really horrible feeling. Also something called flashbacks. Where, people will suddenly feel as if the event is happening again. When people hear talk about if i smoke these of fuel, i think im back on that road in a rack look at the ied explosion. Are a lot of these events. Avoiding social situations. Is, amongething that most people with postTraumatic Stress disorder, but we hear a lot from our veterans, is that they have isolated themselves from their family. They say i do not want my family to know what i have seen and heard. I did not want those pictures in their head that i have in my head. So, unfortunately, there is a sense of isolation. Families feel that they do not want to talk to me anymore. The reality is that they feel they cannot talk to the family anymore. They do not want them to think about what they are carrying around. Impacts not just individual, it impacts the family, it impacts the community. People are not able to go to work in many cases. They have to rely on other sources of income. Many medical causes are associated with the disorder. Veterans with ptsd served in a given year, the iraq and Afghanistan War was 20 . It was 12 in vietnam war. Estimated 30 in their lifetime. So, i wanted to tension which are guess just said, about the key tsd causing people to not want to talk about it. Im wondering, this is not something ive heard anybody say, im wondering if the elevated suicide rate, and the elevated rates of ptsd and this new phenomenon may have something to do with a deep sense of shame. For what is being done over in his countries. The fact that our military is being used like the arm muscle wing of the mob. And are trying to go around use brute force to do what we want to do. Perhaps, the conscience of our soldiers, their understanding, they are perceiving the injustice. Ok. Guest postTraumatic Stress disorder has been around since the beginning of time. Homer actually talked about it. At different times we have called it different things. We call the soldiers heart. We called it warner rosies. Recall that shellshocked. After the vietnam war is when we first got the term postTraumatic Stress disorder. That sometimes in war, people talk about doing things that they would not do if they were civilian, but i do not want to say, and i cannot say that people are committing suicide because of the shame that they feel when theyre over there. We have not heard our military talk about that. We have heard them talk about how things are different over there. But not in the way that the callers talking about. But, depression often comes along with postTraumatic Stress divorce order. Because, if youre always hypervigilant, or en garde. If youre not able to sleep, that makes your body feel very fatigued. You can become very depressed when you cannot find a way to sleep. There is a high suicide rate among veterans. We have to do something about it. PostTraumatic Stress disorder may be one factor. But, it is probably not the only factor. Hear from patrick in oakland california. Active military. Hello. Caller good morning. I am actually a vietnam veteran. I received a few counseling sessions. I first engaged with the health system. About seven years ago. They divide you between combat veterans and active combat, and those who were in support. If you are in support, my feeling is that they tend to put you on a lower level of service or activity. Helpful,e counseling but i also find that survivors guilt is a big component for the web a lot of us feel. That our government to did the wrong thing. I have had 40 years to look back on it. To see what happens in modernday warfare, it just gets worse. Thank you. Thank you. On the firsthink set of studies that we did with our Treatment Program that we were working with Vietnam Veterans back in 1990. We heard some of the same concerns from that group of veterans during that amount of time. So, i do think postTraumatic Stress disorder is something that does not usually go away without effective intervention. And, i encourage people to look around and try to get something to help. Sometimes intervention, which is what were doing will help people. This is a problem that we have to face as a country. Were going to have to put in the resources needed to help people get their lives back. Veterans, our war fighters have put themselves on the line for us. We need to give Something Back to them. That they have not seen a recurrence of ptsd, but are they still being with anxiety and depression . I think there are still dealing with anxiety and some depression. Some people say, you know, i am having a nightmare once a month. Where before, i was only having three or four week. So, i would never say that they no longer have any symptoms of postTraumatic Stress disorder. I would say that if they had symptoms, they are not in differing with their lives. As Mental Health professionals, that is an important consideration for us. That is, are you able to do those things and function effectively in if you have some symptoms remaining. I think people deal with symptoms in different ways. One of the things that we talk about is that we have people talking about what are the goals and values. A treatment zone is behavioral activation. What will be accomplished . Are veterans of the programs onl say we need to focus something that you want to do. Being able to go ahead and get yourself started doing that. It makes me feel better. It makes me feel like i have a goal again. Like i have a reason to keep going in to work with a family and get back to doing those things, and what we find is that when people are behaviorally activated, when they go out and do things that they enjoy, they start to feel some sense of happiness again. Instead of anxiety. The other thing we do is part of a program is just a program where people have to work very actively for the program. So, if you teach these people skills that they can keep using, when they are back in their home environment, so that we can say to them, we have taught you how to do this. This is something that if you have a stressful day, you might need to do it again. So, we give people the tools that they need to be able to manage these things that they can deal with when they go home. She is from the university of pittsburgh. Thatare being mistreated are going out there to help these Vietnam Veterans and all these other veterans. The veterans do not to get to the funds. Through funneled courses like yourself, or through the mayors office. These it veterans do not get the money. Couldnt that be a reason for of theses and the 22 people committing suicide every day . Sometimes including homelessness. Sometimes the inability to get the Financial Support that they do deserve. I would encourage people to search their resources. There probably are veterans groups or tyrannies who would deal with those types of issues. I think for our grant, the money that we have received is setting up a program that actually as testing how Virtual Reality can help. It is a new form. It is not the treatment. It helps make the treatment better. I think you are right. Research and continued advancements need to be given up to people who really need them. That is the challenge that we need to make sure all veterans get the right treatment, and all the treatment that they need. Otherwise they have to go through paperwork and processes. Like you are saying, there is a process to it. I think the thing that people are doing this deliberately, or as i said, deliberately not talking to people, just to be mean, is not correct. So, understanding what the order is, and what the symptoms are can really be helpful for family members. I think the other thing they can do is help look for different resources that might be available. The Veterans Administration is one specific resource that is out there. There are other programs. There are other things that can be done. It is sometimes difficult to find them. Sometimes it takes some time. You have to look for local resources through local veterans groups. We have been running this program for four years. Im still surprised when people in orlando come up to me and say i did not know we had a postTraumatic Stress disorder clinic in orlando. So, we just need to keep at it. I would encourage family members to be there to support their loved ones. Because, that will help people understand what it is they are going through, and family support, just knowing families there and our supply and you is very crucial in helping veterans get the treatment that they need. Treatments such as the ones we are doing is not only pleasant to do, but, if they have family members who are behind them, cheering them on, saying thank you for doing this, because it will make you better. There is a comment that the higher rates of suicide in this compared to previous wars have to do with the fact that that trend is being described in combination and ambien. Drugs thatd sedative sometimes have a risk of suicide. Can you address the issue of Prescription Medication . Well, i can address that from a research perspective. Im not a psychiatrist. So, i want to make it clear that i am not offering any medical advice about what she should take or not take. I think that in general, there are couple things going on here. One is that there are more medications be made available. Sometimes certain medications taken in, nation can be lethal. We do know that. We know that mixing medication ath alcohol can also be lethal combination. The other thing is that it is were talkingr if about more suicides, or there really are more suicides. I want to give you my example that is not related to anyone, there are a lot of times people know that one car accident, sometimes people wonder, was a trillion accident . To meeople will talk about wealth i want to kill myself, ill just run my car into a bridge. Need to be careful about how we classify all of these events. People a day, 10 20 people a day, or one person a day. That is too many. Result of is the medication or other factors, that is too many. We have to turn our resources and stop it. Host the Treatment Program that you have, do you incorporate prescriptions into your treatment . Guest no. This program does not prescribe medication. Some people, some of the people come to the program will come and they are already on medication prescribed by someone else. Them to not change anything while they are in the program. Is still treating me for my ptsd. I had to make medication. And, i still cannot watch tv. I still cannot do a lot of things because dealing with the military, it is upsetting. We are not boots on the ground. We are men. We are women. We are serving our country. I think that they should treat us better than as boots on the ground. That leaves and insensitivity. In my mind, all of our military are heroes. They are going to do something that the rest of us do not do. If youre willing to put yourself on the line to save us back here in the United States. And, the reason i do this program, i tell people that the i am privileged to be able to provide his Treatment Program and this clinic that we have to the men and women who have done so much for us. Host what is the goal of the military giving you this grant . What is the goal . Guest the goal is to see if the have bothat we individual and Group Treatment as well as the Virtual Reality, as well as doing it in a three week version as opposed to what we have a 17 week version which is more typical of the outpatient therapy. The goal of all that is to see if these treatments are effective. With postTraumatic Stress disorder if we can get the success rate higher than other treatments. This in a verydo compressed fashion. One of these things that a lot of people out there both therapists and people the public who think that doing exposure therapy every day for three weeks, that people are going to come in to use the words go crazy, or fall apart. Thatone of these things were proving with the program is that it does not happen. You can have people approach these events in the very concentrated fashion, people get better, they do not get worse. There is also concern that a program that might be this compressed with treatment every day might increase peoples rates of Substance Abuse, or might lead to more suicidal thoughts. Again, we are collecting the data to show that does not happen. Suicidal thoughts archly go down. Even a treatment is occurring every day. These are very important Outcomes Data we are to report. Like illnesses come were people have intense rehabilitation, we can do the same thing for postTraumatic Stress disorder. We need to get that out there. A large part of the program at will provide training workshops. We would get out of orlando florida. Ith the money that we have open to any branch of the military. I think we as Mental Health professionals have to lead the way on this. Disorder because that is the word that is in the diagnostic manual. That is the word we find when we are talking to insurance companies. I strongly support the use of the term ptsd. I think it is something we can all experience, one we talked the public about this, i try to get them to understand, i think they get about where you were on 9 11. You know, did you have after everything that happened in our country that day, did you have trouble sleeping at night . Did you find that you could not stop thinking about the airplanes onto the towers. Did they find the pentagon you go pentagon . Most people can say yes, iron member that. Most people have a traumatic event that they have. Think about if that did not go away. We are all wired differently. So, for example, there are tople who are more reactive large sounds, and loud sound occurs in a Movie Theater, you will see some people jump, some people will sit there and be quiet. Fruit different nervous systems. The closer you are to the event. That youlikely it is will have to deal with longerlasting conditions, because you are there at the scene of where it happened. So, both colors are correct. The third thing we know is that it is often much harder for men to admit that they have depression or anxiety, because we socialize boys from a very early age, or at least we used to, big boys dont cry. Do not show that you are afraid. So, the men have gotten the message that even though they are feeling one way, they have to behave another way. This is one of the reasons why we sometimes see among men higher rates of Substance Abuse substanceson to because men with anxieties and postTraumatic Stress disorder are likely to use our hall as an example to try to decrease the symptoms that they have. They think while i drink, i will relax. Well i drink enough, i will follow sleep. You do not follow sleep come you pass out. But, people are afraid of the stigma. That is something that we as psychologists and psychiatrists and all Mental Health professionals have to get out to the public. This postTraumatic Stress is no different than if you had a physical injury from this war. Some of these things that have happened in people over there and again, even with crime victims here is country some of the things that happen to people, some of the things that happen to be have been sexually assaulted, these are horrific things. Nobody should be expected to just snap out of it. So, we have got to do a better job of convincing everyone that this is just another form of injury. Debilitating,t as it is every bit as needs to be recognized as something that people are not just in many cases, facing. They are not just lazy. They cannot just pick themselves up and do it. We have to get to that point. I hope that programs like this with you, that we can reach the people that it really matters too. About the modernize treatment that you are doing at the university. It is the only comprehensive one. Other places like that the a and look military institutions at Virtual Reality therapy. There are other places that have the Virtual Reality therapy. There are some the as where it is set up and functioning. All of the vads and the conditions of the views have been trained by the v. A. Into treatments that do have some success. One is what is called prolonged exposure. Which is the same principle of exposure but without the Virtual Reality. Another is called cognitive processes therapy. It seems to have some effectiveness for what we call in the v. A. What is called military trauma. Both have treatments that have some effectiveness that are available in the v. A. They do not have the Group Components that we have. They are not always use the Virtual Reality. They do not do it in this compressed fashion.
