Registration must begin with a phone call to the agency: (844) 684-6333 or text-to-phone/TTY at (800) 462-7585 from 9 a.m. to 9 p.m. Eastern Time, Monday through Friday. Multilingual operators are available. A full application, submitted online or in the mail, must include a death certificate that indicates the death was related to COVID-19. Expenses that are covered (and must be documented) include the transfer of remains, a casket or urn, a burial plot, a marker or headstone; and the cost of clergy and a funeral service. Life insurance proceeds won't disqualify an application.
