不少人在接种第2剂新冠疫苗后，副作用表现得更明显、严重，加拿大专家指出这很正常，是抗体与免疫系统双重发挥作用的结果。
加拿大环球新闻（Global News）报导，加拿大女子杨蓓佳（Becca
Young，音译）表示，自己接种完第2剂疫苗后隔天几乎无法起身，她说手臂、背部都酸痛，非常痛，还有头痛，就像宿醉。杨蓓佳第1剂打的是辉瑞/BNT疫苗，第2剂打莫德纳疫苗，打完第1剂时只有手臂酸痛的症状。
报导指出，加拿大有许多人像杨蓓佳这样，第2剂后症状更明显，不过专家表示这很正常。
加拿大渥太华大学（University of Ottawa）神经免疫学、压力及内分泌学实验室（Neuro-Immunology
Stress and Endocrinology Laboratory）主任伊斯梅尔（Nafissa
Ismail）表示，在第2次接触到同一个病原体时，人体的免疫反应通常不会同样剧烈，但是新冠疫苗作用方式不同。
他解释mRNA或病毒载体（Viral vector）疫苗指引细胞侦测出冠状病毒的棘状蛋白（spike
protein），接种这类疫苗并不是像一般其他疫苗那样，将已经弱化或是死掉的病毒注射进人体内，所以人体第一次接触到讯息时，”我们的免疫系统还没有制造出抗体。”
“但是当我们接种第2剂疫苗时，就有双重作用，已经有部分抗体就绪准备上战场，再加上我们要求免疫系统制造更多抗体，所以症状往往会被放大。”
加拿大西安大略大学（Western University）免疫学家克富特（Steven
Kerfoot）表示，第2剂疫苗后的副作用更强的说法还没有被证实，不过他个人的经验确实也是如此，他第1剂接种AZ疫苗，第2剂接种mRNA疫苗，他认为混打疫苗可能会让部分民众有更强烈的副作用反应，不过如果2剂都接种mRNA疫苗，可能就不会出现较强烈的副作用。克富特解释，民众在接种第1剂疫苗后几个小时至几天内出现的轻微不适感觉，是因为免疫系统”开始对病毒感染感到怀疑”，并且开始抵抗。
如果民众感到疼痛、疲倦，那代表身体在向你宣告该休息了，以保存能量；如果发烧，代表身体正在抑制病毒的复制能力。他说无论是接种第1剂或第2剂，民众都可能出现上述副作用，差别在于第2剂后体内出现了抗体等特定的传感器，所以民众接种第2剂后会有更多症状是相当合理的，因为额外的抗体检测系统已经准备好更快、更强烈地做出反应。
关于第2剂疫苗的副作用，英国政府在官网指出，接种第2剂AZ疫苗的副作用较第1剂更轻微，第2剂辉瑞/BNT或莫德纳疫苗的副作用会比第1剂更强烈，研究也指出，第1剂施打AZ疫苗后，第2剂换打BNT疫苗或莫德纳疫苗，出现副作用的风险也会比较高。
