델타도 버거운데…람다 변이 확산, 페루선 확진자의 80%

페루에서 최초 발견된 신종 코로나바이러스 감염증(코로나19) 람다 변이가 남미를 중심으로 확산하면서 델타 변이처럼 전 세계적으로 확산하는 것 아니냐는 우려가 나온다.
13일(현지시간) 세계보건기구(WHO)에 따르면 람다(C.37) 변이는 지난해 12월 남미 페루에서 처음 확인됐다. 다른 바이러스와 마찬가지로 코로나19 바이러스도 끊임없이 진화하고 무수한 변이를 만든다. 모든 변이가 위험하거나 의미 있는 것은 아니어서 WHO는 기존 바이러스 대비 전파력이나 치명률 등에 변화가 있는지, 백신이 효과가 있는지 등을 고려해 '우려 변이'와 '관심 변이'를 지정해 관리하고 있다.
현재 영국, 남아프리카공화국, 브라질, 인도에서 발견된 변이들이 차례로 알파, 베타, 감마, 델타로 명명돼 우려 변이로 지정돼 있다. 그보다 한 단계 낮은 관심 변이로는 지난달 14일 추가된 람다를 포함해 에타, 요타, 카파 등 4종이 있다. 기타 감시 대상인 변이도 10여 종 있다.
람다 변이의 경우 관심 변이로 지정되기 전엔 '안데스 변이'로 불렸다. 페루를 비롯한 남미의 코로나19 재확산이 심각했던 지난 4월 브라질발 감마 변이와 함께 새로운 안데스 변이의 존재도 주목을 받았다.
지난달 WHO에 따르면 4월 이후 두 달간 페루 신규 코로나19 확진자의 80%가 람다 변이에 감염됐으며, 아르헨티나와 칠레의 람다 변이 비율은 3분의 1가량이었다. 지난 4월 확진된 알베르토 페르난데스 아르헨티나 대통령이 감염된 것도 람다 변이로 알려졌다. 당시 페르난데스 대통령은 러시아 스푸트니크V 백신을 두 차례 다 맞고도 감염됐는데, 백신 덕에 증상이 가벼웠다고 말했다.

현재 남미 각국과 미국, 캐나다, 유럽 일부 국가 등 약 30개국에서 람다 변이가 확인됐다. WHO가 관심 변이로 지정한 것은 기존 바이러스 대비 더 위험할 가능성이 있다고 본 것이지만, 아직 람다 변이의 전파력 등에 대해 정확히 연구된 것이 없다.
페루의 인구 대비 코로나19 사망자가 전 세계에서 가장 많고, 치명률도 9%로 세계 최고 수준이지만 이를 근거로 람다 변이가 더 위험하다고 보기는 어렵다. 페루의 인구 대비 사망자는 람다 변이가 확산하기 전인 지난해 8월에도 세계 최다였으며, 역시 람다가 퍼진 칠레의 치명률은 2.14%로 세계 평균(2.16%) 수준이다. 페루는 4월 이후, 칠레는 6월 이후 코로나19 확산이 주춤한 상태다.
미 일간 뉴욕타임스(NYT)는 지난 8일 "람다가 다른 변이보다 더 전파력이 높은지, 감염자의 증상이 더 심하고 백신의 효과를 감소시키는지는 분명치 않다"고 분석했다.
람다 변이를 연구한 너새니얼 랜도 뉴욕대 미생물학 교수는 NYT에 "람다 변이가 델타보다 더 위험한 것이라고 생각할 근거는 없다"며 "이 변이에 대해 더 알기 전에 미리 걱정할 이유는 없다"고 말했다.
랜도 교수와 칠레대 리카르도 소토리포 교수 등은 아직 정식 게재 전인 최근 연구 논문에서 화이자, 모더나, 시노백 백신으로 만들어진 항체가 람다 변이에는 덜 강력하지만, 여전히 바이러스 중화는 가능하다고 말했다. 아울러 항체 외에 T세포 등도 면역 작용을 하기 때문에 소토리포 교수는 "중화항체의 감소가 백신 효과의 감소를 의미하진 않는다"고 설명했다.
미국 프레드허친슨 암연구센터의 진화생물학자 트레버 베드퍼드도 "람다가 출현한 지 꽤 지났지만 감마 변이만큼도 미국에 심하게 침투하진 않았다"며 "델타 변이에 관심을 집중해야 한다"고 말했다.
다만 아직 코로나19 바이러스 자체에 대해 확인되지 않은 것이 많은 만큼 람다를 비롯한 새로운 변이들에 계속 관심을 두고 주의해야 한다고 전문가들은 지적한다.

Men appear more likely than women to experience severe outcomes from COVID-19 regardless of whether the men have underlying health conditions, according to a new study. Researchers in New York City who tracked roughly 5,000 patients of both sexes who were diagnosed in early 2020 found that men were sicker when first diagnosed and had a higher need for intensive care treatment and higher rates of death than women. This was true even though the men on average were younger and less likely than women to have risk factors such as obesity and high blood pressure that previously have been linked with poor COVID-19 outcomes. Among women, the likelihood of severe COVID-19 outcomes did seem to be related to their health status, the researchers said in a report published on Wednesday in the journal Communications Medicine. A second analysis of another group of roughly 1,600 patients in New York City diagnosed later in the year showed similar patterns. The authors noted that because their information on patients’ pre-existing medical problems may have been incomplete, larger studies are needed to confirm their findings and to explain the causes.
