Osler's valediction: how might physicians contribute to the effort to postpone human extinction?

In August 2023 the editors of 11 leading medical and health journals called for the elimination of nuclear weapons.1 Other anthropogenic threats to humanity include the climate emergency, biological and chemical weapons of mass destruction, hazardous uses of gene editing, and uncontrolled artificial intelligence. Existential risk studies (ERS) and its corollary, existential ethics, are emerging forums for interdisciplinary dialogues about these risks to human survival.

Although International Physicians for the Prevention of Nuclear War (IPPNW) has focused on certain anthropogenic threats to humanity since the 1980s, physicians are conspicuously absent from the more recent, comprehensive dialogues. Thoughtful papers on the risk of human extinction published more than a decade ago drew little notice from anyone.23 Major treatises of ERS and existential ethics do not include medicine in their indices.45 These observations may reflect domination by disciplines such as philosophy, economics, and law. How might physicians contribute to the effort to postpone human extinction?

Alison Whiting, policy officer for standards at the General Medical Council, says, “In our core guidance, Good Medical Practice , we say that wherever possible doctors should not provide medical care to anyone with whom they have a close personal relationship.

“There are several reasons for this. It can be difficult for patients in these circumstances to be open and honest about their medical history and treatment, and doctors may come under pressure to provide a particular treatment or find it difficult to be objective about a patient’s need for it.

“We don’t forbid doctors from treating their family or friends if it may be reasonable for them to do so, where no other doctor is available—for example, in an emergency or if they work in a remote rural area. But we do urge caution and careful consideration of the risks, as well as the alternatives available for meeting the patient’s needs. We also impress the importance of record keeping, which can sometimes fall by the wayside when relationships are both professional and personal.

Do doctors need to be told to be kind?

On 22 August 2023, the General Medical Council (GMC) published the latest version of Good Medical Practice , its guidance for doctors.1 The guidance will come into effect on 30 January 2024, but it has already been widely debated by doctors on social media. The target of the discussion is the introduction of a requirement to treat patients and colleagues with “kindness, courtesy, and respect.”

Critics of the guidance say “kindness” is too subjective. They say it is patronising. They say it will prevent doctors from speaking out, or sharing unpleasant news. They say it will result in frivolous complaints and referrals to the GMC, with a disproportionate impact on women and doctors trained abroad.

The GMC, at paragraph 23 of its guidance, elaborates on the meaning of “kindness, courtesy, and respect.” It means that …

There’s currently no shortage of controversies in medicine, with repeated strikes by doctors and proposals to shorten training times and introduce more non-doctors into the workforce. These matters have generated strong feelings. The result is an abundance of criticism on social media by doctors towards each other and other clinical colleagues, be they physician associates or nurse practitioners. What might once have appeared only in the pages of journals or only been uttered in medical meetings is now on social media for all to see.

There may be benefits to such public venting, from “getting things off one’s chest” to promoting a cause to a wide audience, but the harms are considerable. The discord and acrimony tarnish the image of a profession known for its dignity, civility, and common purpose in promoting patients’ interests. …
