I've owned and operated Walsh Pharmacy in Fall River for more than 50 years. Over that time I've met thousands of residents and their families. Serving them has been my pleasure.
For many of my customers, I am their only healthcare provider. They've come to trust and rely on me. As an independent pharmacist, you really get to know your patients, their medical histories, and their fears and concerns surrounding medical conditions they have. Sometimes it can be a matter of life and death.  
For instance, several years ago I had one couple come to me for substance abuse treatment. Both had high powered jobs in their town and both were addicted and desperate. We talked for quite a while. I was able to get them each a 3-day supply of suboxone medication.  Grateful for the time and help I'd given them, they returned to say they were only going to come to my pharmacy from now on for all their medication needs. There are hundreds of similar stories I could tell you about, especially concerning patients with debilitating diseases like HIV, depression, mental illness and many more. It's the personal relationships I form with them that is so very important. You don't get that with the big chain pharmacies.

Independent pharmacies struggle with low payments, confusing fees

Whole Health is the only independent pharmacy on Cape Cod, an increasingly rare type of business that a study released last month indicates has been facing an uphill fight in a David vs. Goliath battle.
The study by the Massachusetts Independent Pharmacists Association concluded that reimbursement to the state's independent pharmacies was inadequate. Loopholes in the overall Massachusetts Managed Care Program, mandatory steering of customers to mail-order specialty pharmacies, low dispensing fees and retroactive discounts have put independent pharmacies in a financial bind, according to the study.
Furthermore, it found that taxpayers were overcharged an estimated $25 million for MassHealth Managed Care claims from 2018-19.

The games PBMs play

A report commissioned by the Massachusetts Independent Pharmacists Association found PBMs in the state’s Medicaid managed care program are playing the all-too-familiar games that drive community pharmacies out of business while costing taxpayers millions of dollars. An analysis of pharmacy reimbursement data found PBMs pocket a 33 percent markup per prescription in spread pricing, which comes out to about $25 million a year.
