The Greater Manchester fire and rescue service has apologised for its response to the Manchester Arena attack three years ago, saying it was neither adequate nor effective. At the Public Inquiry the lawyer representing Fire Fighters admitted that it was unacceptable that it took two hours for them to arrive at the scene. Our correspondent Judith Moritz reports.
