Needs assessment highlighted the importance of Case Management in the community. It clearly noticed Service Providers identified a need for increased capacity and consumers provided difficulties of the navigation. They will conduct an analysis of the citywide Case Management system to identify unnet needs and gaps in services and assess streamlining. They will work closely with dos and communitybased partners to provide the department with a comprehensive systems assessment report.
