Metro rail cars that were sidelined in November after multiple incidents of cars separating while in service will stay off line, safety inspectors said Tuesday.
The Washington Metrorail Safety Commission said the 6000 series cars, a nearly 20-year-old model that made up about 15 percent of Metro’s active rail cars, will be out of service indefinitely because of problems with how parts were replaced during a recent overhaul. Safety inspectors said the repairs led to rail car separations on Oct. 9 and Nov. 24, both of which occurred on the Red Line. ....

Metro tells STATter911 its investigations identified safety lapses during two separate rail emergencies near the Fort Totten Metro Station. In one case, Metro workers did not use a warning device to confirm third rail power remained off during the evacuation of two stranded trains. In another, Metro’s Rail Operations Control Center (ROCC) failed to give accurate information to DC 911 after a worker was hurt inside a tunnel. This adds to a long list of safety concerns identified by both Metro and the Washington Metrorail Safety Commission over the last 16 months. ....

A passenger on a stranded subway train was the first to alert DC 911 to the emergency, further highlighting a serious communications breakdown at Metro’s Rail Operations Control Center (ROCC). The new information from DC’s Office of Unified Communications (OUC) comes two days after the Washington Metrorail Safety Commission’s (WMSC) CEO admonished Metro for its poor communications with police, fire and EMS during the March 26 incident. An OUC timeline and radio traffic recorded by OpenMHz.com illustrate continuing problems at both ROCC and OUC in promptly and accurately dispatching DC Fire & EMS to rail emergencies. ....

The General Manager and CEO of WMATA is issuing immediate policy changes after an unreported rolling event was discovered during an incident investigation.
In a memorandum dated Saturday, Paul Wiedefeld wrote to the Board of Directors he has taken both immediate actions and developed a number of larger organizational changes to the way we manage incidents that will apply to all rail, bus, paratransit and certain police incident responses.
According to the document, WMATA investigated a disabled train incident from March 26 involving the Red Line near Rhode Island station platform. Around 109 passengers were on the train at the time of the stalling and were eventually evacuated. ....
