Mr macron has already said he will work to strengthen the eu, and eu leaders have rushed to congratulate him. They see mr macron giving the eu new impetus. So this win means the uk is about to negotiate brexit facing an eu starting to feel confident that the Populist Tide may be turning. Lets go to paris and our correspondent Hugh Schofield is there. What does this new president presidency mean to the uk and its brexit negotiations. Two things to say, the 1st, he will be seen as a good thing by the British Government because he is generally pro british, he speaks english, he knows the city, a former banker, his instincts are free market, all of these things are good buttons to press as far as london is concerned. The other side of the coin is that hes firmly pro european and his ist instinct is going to be to patch up ist instinct is going to be to patch up the Franco German relationship and try to re establish that as the core of europe and to make europe a functioning, effective dynamic organisation. And in that sense, and in that context, his furry opposed to brexit which he has said is a big mistake and he will not want to lend any hostages to fortune by being too generous to britain on the way out. The Royal College of midwives says Maternity Services are heading towards a crisis because of demands on the services. 0ur reporter divya talwar has been to meet some of the families affected. As an expectant parent, youre excited to become a parent for the first time. Im angry, and im always going to be angry, because theyve taken my sons life away him. Taken my sons life away from him. There is no reason why he shouldnt have been here with us today, and i have to live with that for The Rest Of My Life. Every day i have to live with the fact that i am a victim of the nhs. You think you can trust these people, and you are meant to, but theyve let us down. Theyve let us down big time. There were six hours that my son suffered, and there were four opportunities that were missed to save his life. When i found out i was pregnant almost eight months ago, i did a lot of research looking into the hospitals around me to try and find the best place to have this baby. But all of the Maternity Units in my area were either inadequate or in need of improvement. So i had to find a place outside of my local area, which isnt ideal, but at least the Maternity Services are meant to be a bit better than what ive got around me. 0ther mums to be may also find themselves in a similar situation, with around one in three Maternity Services in england rated inadequate or Requiring Improvement by the regulator. Last month it was discovered that at least seven babies had died in avoidable circumstances in less than two years at shrewsbury and telford nhs trust. It led englands Health Secretary jeremy hunt to order an investigation, and the trust has since paid out millions in compensation after similar mistakes led to babies born with Brain Injuries. But weve found that in other Maternity Units across england there are also cases where serious mistakes are being made. Come in. This is jade penney, shes 26 and a single mum with two children. Lucas has got cerebral palsy, which means he cant walk, talk, see and hear very well. Do you want to high 5 me . Good boy jades lawyers argue that lucas brain damage is due to a lack of oxygen when he had his Incubation Tube replaced. The nhs trust is defending the claim. When did you find out that something wasnt quite right . Pretty much straight away, but we didnt know this obviously for a long time, so a couple of days after he was born. So, he was born on the fifth, and on the seventh they were re tubing lucas, so they had re tubed him in the morning without any problems at all, and then they re tubed him again in the afternoon. As they re tubed him, lucas basically put up a fight, he was moving his head to and from. Altogether it took them 20 minutes to re tube lucas. Imagine laying down and not being able to breathe, but you cant tell someone and youre awake. It must be the most horrible thing ever to go through, and he couldnt tell anyone. And i think thats just what upsets me the most. Jades now taking legal action against the nhs trust. Yeah, emotionally it plays on you a lot. It plays on me a lot when were out and i see kids his age, thats when i think it really gets to me. Take, for instance, if ive gone to the park with lucas before and ive seen kids his age, theyll kind of avoid luke, not because theyre being horrible but theres not that much known to kids when theyre younger about disabilities, theres not at all. Sometimes theyll look at him and they get a bit scared and they wont go up to him, and all lucas wants to do is go and say hello. Whats the hardest aspect of all this . Knowing that hes got this for the rest of his life. Because not only does it affect me, its affecting his life, and his quality of life. Yeah, hes still alive, but he hasnt got the quality of life other children without disabilities his age have, so he will miss out on a lot. Hes going to. And he knows himself when he sees kids running around that he cant do that, so it impacts him as well as me, which is sad. Its sad for him to pick that up. And its not his fault, and thats whats horrible. Its not his fault at all. He is like the Innocent Party in all of it, you know . Arent you . Serious or adverse incidents where an unexpected harm, injury or death has happened. It could be anything from records being lost to a mother or baby dying. Last year alone, there were 220 mistakes recorded every day. Weve also found that nearly 260 mothers or babies died overfour years. These deaths were either unexpected or could have been avoided. Only 39 out of 81 trusts responded to this question, so the number of deaths could be much higher. Some of the other incidents weve heard about include staff shortages, wrong medicines being given to patients, records being lost, and delays in care. Its those delays in care that can be the difference between life and death. Sarah ellis and her fiance adam lost their baby in 2014. When i first fell pregnant, everything was amazing, wasnt it . Yeah. We were absolutely over the moon. Couldnt wait to take him to the football with me, and be a Huddersfield Town fan like his dad. Sarah was left waiting for hours on a busy maternity ward, despite telling staff that she couldnt feel her baby moving, and there were Warning Signs of infection. We were left for six hours. We didnt really know anything. Theyjust told us, well, theyjust reassured us, didnt they . Everything, you know, everythings 0k. When a qualified midwife says to you, it could be that, maybe, possibly, were not sure, i wasnt really concerned because if there was anything alarming they would be doing something more. The inquest later showed sarah should have had an emergency c section many hours before she did. Medical staff failed to act on Warning Signs, and their son gino was severely starved of oxygen. One of the doctors that was there pulled me to one side and just said, hes not in a good condition, he was born in a really bad condition and if he does pull through, hes going to be very badly brain damaged. I was in the corridor and i was with her mum and dad and ijust said to her mum and dad, isaid. I said, how am i going to tell sarah that hes not all right . Gino was placed on a Life Support Machine, butjust days later sarah and adam were advised to withdraw treatment. The words that were used was he was unrecoverable. That was when we knew he wasnt going to get any better. We had to make a decision. We had to make a joint decision, yeah. We had to make a decision for him that, you know, we would turn the machines off. Its a conversation you never think youre going to have to have, is it . No. How do you discuss how youre going to end your sons life . The couple decided to take legal action. A coroners report found the hospital missed four opportunities to save ginos life. Everybody makes mistakes, i do, we all do, but to see so many people make so many different mistakes within six hours is just shocking. People who you put your trust in, your life is in their hands, and ginos life was in their hands and they didnt take care of him. Every single day i think about him. Every single day i think, why, why us . We have to live with the fact that we are a victim of the nhs. So many babies have lost their lives, and so many families have been destroyed because theyre not doing theirjob right. Sarah and adam got compensation from the trust. The nhs spends hundreds of millions on Compensation Pay Outs for blunders made by Maternity Staff. In 2015 it paid out more than £0. 5 billion. So whats going wrong . Reviews into englands Maternity Services have found they re under pressure. So ive gotjust over three weeks before this babys due, and i had my latest check up with the midwife earlier today. Over the course of this pregnancy ive been seen by different midwives at almost every appointment ive had, and at times ive had to wait up to two hours just to be seen. The Royal College of midwives said safety is being compromised because of the pressure our Maternity Services are under. Its my view that were heading for a crisis in our Maternity Services. The simple truth is, we do not have enough midwives working in them right now. Were also seeing more midwives heading for retirement, were seeing more midwives leaving the profession because of stress, and were seeing a slight reduction in the number of student midwives training. Alongside that, weve got much more complexity in pregnancy, which takes more care, and weve got an increasing number of births in this country. So if you put all that together, as opposed to only having one of these things going on, we would say thats looking like a bit of a crisis. What do you think needs to be done to make our Maternity Units safer . We need to reduce the number of mistakes in our Maternity Services to an absolute minimum. And if were going to do that, our Maternity Services have to be properly staffed. We cant deliver the safest possible care if we dont have enough midwives and enough doctors working in our services. Could you get me the Emergency Box . Thank you. Theres going to be lots of us doing lots of things all at the same time, sojust bear with us. Libbys actually one of the doctors at the hospitals maternity ward. I think we can call that a wrap, as we say, and welljust have a chat about how we did. I think libby stole the show, very good acting yes, i know, absolutely the hospitals pioneered this training, known as prompt, where midwives, doctors and other staff train together and simulate emergencies they may face on their maternity wards. Everything else was very clinically done in a very straightforward order. The Health Secretarys announced a number of measures to improve the safety of Maternity Care in the nhs. They include more money for Training Courses like this, and a fund to pilot new ideas for improving maternity. For sarah and adam, the mistakes leading to ginos death have left them worried to have another baby. We do want more family, but when that time comes i really dont know what were going to do. Where do we go . Who do we trust . We went in with a baby seat and came out with a death certificate. You think you can trust these people, and you are meant to, but theyve let us down. Theyve let us down big time. For The Rest Of My Life im going to be angry, and ill never, ever forgive anyone for that, you know . Lisa says, the nhs Midwifery Team at Epsom Hospital were fantastic for the birth of my first baby 11 weeks ago. Brilliant care and fantastic facilities. I feel very lucky and incredibly grateful. And the moment is defined by the fact that even though he won this big majority, most people, you know, dont feel a huge affinity towards him. This is worrying me as a doctor who is near the end of his training about that. Well, the worst thing that i have seen recently is that for the First Time Since 2003 the death rate amongst children under one is going up. Thats going up in a country thats the fifth biggest economy in the world. We have an under one Mortality Rate that is 15th out of 19 in western countries. Why do you say thats happening . Your message to politicians from whichever party would be in this Election Campaign . I think training up Election Campaign . I think training up future Healthcare Professionals is really important. Thats our sort of long term vision. Bursaries were cut a few years ago now and personally, without a bursary, i was one of the last to go through with a bursary scheme, i could not have afforded to train and i would not have qualified and be in working for an nhs trust if that wasnt the case. And in sort of the last year there has been a 23 drop in Nursing Applications across the board and thats also overseas students applying to come and train here so thats really concerning especially. Whats the knock on effect of that going to be, do you think . That drop in Nursing Applications . Well, i think it will have a knock on effect. Quite clearly, i think there are a couple of general points here that the doctors, the Healthcare Professionals, nurses included, they need to be empowered and they need to feel that they are appreciated. 0k. And to feel that they are appreciated. Ok. And its quite important that we cant just rely ok. And its quite important that we cantjust rely on goodwill all the time because the doctors and the nurse love the National Health service and they are there could do a greatjob for this institution, we cant just rely a greatjob for this institution, we cantjust rely on the goodwill and we need to make sure they are partners in the nhs and the second point is that what hugh said, we need people from outside and i think we have more than 50,000 european nationals working in the nhs at the moment as doctors, nurses and researchers and we need to think about that when were talking about brexit and how we retain them. Just on that first point. I think its really important to pay and value our staff well, but going further, the nhs is actually making its staff sick which i find quite frightening. Depression and post Traumatic Stress disorder and anxiety is rife. Doing paediatrics and looking after sick and Vulnerable Children is very difficult, but having a 2 and a half hour shift where you have to break, people fall asleep at the wheel, its really quite scary. No wonder people are being scared off. Has that happened to you, 12 hour shift, driving home, falling asleep . I felt too tired getting into my car andnded up having to get a taxi home and a few hours previously i was making life and death decisions with sick children. Youre going to become a junior government two months after the next government two months after the next government gets in. What are the challenges for you speaking to friends who are starting as Junior Doctors, maybe theyve done one or two years, the sentiment coming from them is theyre holding off applying for further training. Hugh, we know about what some parties are saying they would do with the nhs and with social care . Well, labour, last week, said they would raise the cap on Public Sector pay. Currently 1 , set by the conservative government to go through to 2020. They would raise it to allow pay increases of above that, more in line with inflation. They say they would pay for that by raising Corporation Tax after cuts over the last few years. But actually they werent very clear about how much money it would cost and whether Corporation Tax receipts had been pledged to other policies. The Liberal Democrats said at the weekend they would put a penny on income tax, a penny in the pound, that raises in total £6 billion over the next, by 2020, you get there. The trouble is, how far will that go given the pressures of keeping up with demand and this raises a bigger question does the nhs and social Ca Re Question does the nhs and social care need rather more than what the parties are talking about . An investigation by this programme has found there are at least 1,400 mistakes made in englands nhs Maternity Units every week. The true figure could be much higher because only 81 out of 132 trusts in england responded to our Freedom Of Information request. The most serious incidents include the avoidable deaths of mothers and babies as a result of errors by midwives and doctors. The Royal College of midwives says Maternity Services are heading towards a crisis because of demands on the services. We played you our full report earlier. Heres a short extract. While hundreds of thousands of women give birth safely in Maternity Units across england, there are cases where serious mistakes are being made. Sarah ellis and herfiance, adam, lost their baby in 2014. We were left for six hours, we didnt really know anything. Theyjust told us, well, theyjust reassured us, didnt they . The inquest later showed sarah should have had an emergency c section many hours before she did. Medical staff failed to act on Warning Signs, and their son gino was severely starved of oxygen. I was in the corridor, i was with her mum and dad, and ijust said to her mum and dad, isaid. I said, how am i going to tell sarah that hes not all right . Gino was placed on a Life Support Machine but, just days later, sarah and adam were advised to withdraw treatment. Its a conversation you never think youre going to have to have, is it . No. How do you discuss how youre going to end your sons life . The couple decided to take legal action. A coroners report found the hospital missed four opportunities to save ginos life. Everybody makes mistakes, i do, we all do, but to see so many people make so many different mistakes within six hours isjust shocking. People who you put your trust in, your life is in their hands, and ginos life was in their hands, and they didnt take care of him. We have to live with the fact that were a victim of the nhs. So many babies have lost their lives, and so many families have been destroyed because theyre not doing theirjob right. Sarah and adam got compensation from the trust. An investigation by this programme has found that at least 1000 mistakes are occurring in englands nhs Maternity Units each week. Weve also found that nearly 260 mothers or babies died overfour years. These deaths were either unexpected or could have been avoided. Only 39 out of 81 trusts responded to this question, so the number of deaths could be much higher. The Royal College of midwives said safety is being compromised because of the pressure our Maternity Services are under. Were heading for a crisis in our Maternity Services. The simple truth is, we do not have enough midwives working in them right now. We cant deliver the safest possible care if we dont have enough midwives and enough doctors working in our services. When you go in to have a baby, you expect, you go in with a baby seat and you come out with a baby. But we went in with a baby seat and came out with death certificate. Lets talk now to James Titcombe whose nine day old sonjoshua died as a result of mistakes in the Maternity Unit where he was born. Rachelle mahapatra, a solicitor who specialises in cases where babies have been left with brain damage. Dr pat o brien, a consultant at the Royal College of obstetricians and gynaecologists. Welcome all of you, james, firstly, your reaction to our investigation that over a thousand mistakes are being recorded in england paternity units over the next week. We need to encourage organisations to report incidents and we need to be careful how we report this but we need to find out why theyre happening, are there any themes that represent systemic risks . Pat obrien, how do you respond . Its a terrible tragedy from a couple lose a baby like this andl from a couple lose a baby like this and i heart goes out to them. I think its and i heart goes out to them. I think its 1 of the reasons why the royal oak at has set up each baby Counts Campaign, and it aims to reduce the number of stillbirths by 50 over the next few years and we do that by looking at the investigations in morkel hospitals, look at the lessons learnt and roll them out across the nation and i think that will make a big difference. The most common thing that you see in cases of mistakes in Maternity Units, what would you say . I would say basically the main concern is that the investigation procedures within hospitals are not deriving enough information and that is the only way that hospitals can learn to actually plug the gaps. There used to be consistency of completion of the investigation of reports. Its not thorough enough, diligent. They are diligent enough in some of the trusts but its not consistent and james inquiry, following the sad death of his son found that its not been implemented and so we are in a situation, if theres going to be a National Training scheme, which is going to help to improve the experience for patients and may cause people safer, and stop these people dying, unnecessarily or 129 average annual admitted Brain Injuries in children that we are seeing from the nhs statistics, then things are not going to improve. We need to basically have as much transparency as possible. Absolutely, 1 of the 1st reports of the each baby counts programme find that, for the serious incident investigations were not thorough and a large proportion were failing to involve the parents and i think that was your major point. Absolutely. Joshua died in 2008, of a preventable infection that could have easily been treated at what happened afterwards was even more ha rd happened afterwards was even more hard for us because critical records went missing, the Investigation Report that was done wasnt worth the paper it was written on and very sadly afterjoshua died, mothers and babies continue to be at risk up that trust and a number of other babies died for similar reasons. And looking back even before that, the that that unit that unit that should have been an opportunity to learn was in 2004, that wasnt investigated properly and the whole cascade of events carried on. The point about needing to properly investigate and learn and share that learning across the system is important. So if 25 of cases arent investigated is that a lack of skills amongst those carrying out the investigation, is it a lack of will because they dont really want to know . I dont think it is trying to know . I dont think it is trying to hide things, i think it is a lack of training and a lack of rigour in the process. Within of the major benefits of this each baby Counts Campaign will be to look at this and point out that rigour isnt uniform and the first step will be to make sure the serious incident investigations are being carried out ina investigations are being carried out in a thorough and open and inclusive way throughout the country. When it comes to compensation for Brain Injuries, for example, some of the pay outs can be between £10 million and £15 million, cant they . Yes. How is it assessed to that level . We have a cohort of children who have preserved intelligence, but they are very physically disabled so there are short bursts of brain damage and they cant speak and they have no physical movement, but they will have quite a normal Life Expectancy so they cost a lot of money to care for. Thats the way we calculate the claims. They can go to school and go to university and they want to actually engage in normal life and independence as much as they can. Technology costs a lot of money. So to be able to operate a speech syste m to be able to operate a speech system we are looking at £25,000 every time you renew that piece of equipment. The wheelchairs are expensive, it is right that people should have an independent and fulfilled life as possible. Well, it still affects us every day. We are not finished of all the investigation processesment even though its nine years later, we have got hearings happening. The saddest thing for me is the themes that happened injoshuas case which was midwives and doctors not working together as one team, failure to risk assess properly, those themes delayed intervention. Families still contact me today having recent tranlg daorks i cant emphasise teamwork, working together, training together and learning from things that go wrong are so important and we have not made enough progress in those areas. Why are hearings still going on nine years later . Well, u nfortu nately going on nine years later . Well, unfortunately this is what the process does and things get in the way so afterjoshua died there was a police investigation, the nmmc processes got put on hold and then there was an investigation and everything got put on hold and when that happens things cascade and there is one case relate to go joshua thats still on going and its traumatic for everybody. It is desperately sad for us and desperately sad for us and desperately sad for the staff involved. We have got to get to the point where we can learn quickly and be open and honest and understand what went wrok and put things in place to stop that from happening again.
