An inmate assaulted a chaplain at the Arizona State Prison Complex in Florence Thursday, according to the Arizona Department of Corrections, Rehabilitation, and Reentry.

Less than a year after two men escaped a Florence prison, another Arizona prison had a near miss, as an inmate disappeared and jumped two fences in an apparent escape attempt, according to a report obtained by ABC15.

"Catastrophic" security failures are exposed in a video created by Arizona prison officials to retrace the steps of two sex offenders who escaped Florence’s prison complex last year, according to past and present correction staff.

Using newly obtained reports and videos, ABC15 retraced the steps of two Arizona prison escapees and found that the successful capture of the fugitives led many people to overlook the ‘catastrophic' prison security failings during the 2021 escape.
