New partnership bring health services to Walnut Square residents A new partnership aims to bridge the gap between health care services and social determinants of health in the Walnut Square apartment community in New Orleans East. New Orleans East Hospital and Social Health Bridge, an organization founded in 2019 by Missouri-based Centene Corporation to address social barriers to health, said they will visit residents to offer health screenings, telehealth assistance, home wellness checks, chronic disease management, education and other help. “As a result of this partnership, patients and their family members will have increased access to healthcare services and a wide variety of community focused services supported by the onsite community health worker,” said Dr. Takeisha Davis, CEO of NOEH. “Patients can rest assured knowing they have access to the high-quality patient-centered care New Orleans East Hospital is known for providing.” ....
