Bis ein Biofilm entsteht, durchleben Mikroben mehrere Phasen: Zunächst siedeln sich einzelne freischwimmende Bakterien zufällig locker auf einer Oberfläche wie etwa einem Mineral, Metall oder Öl an. Die Oberfläche muss sich im Kontakt mit Wasser oder feuchter Luft befinden und den Mikroben Nährstoffe bieten. Am häufigsten siedeln sich Bakterien daher an der Grenzschicht einer festen Oberfläche mit Flüssigkeiten an – beispielsweise auf nassen Sterne, aber auch in medizinischen Kathetern.
Unter den dortigen Voraussetzungen werfen einige Bakterienarten zunächst ihr Fortbewegungsmittel, die Geißel, ab und nutzen fadenförmigen Anhängsel, die Pili, zur Anheftung an die Oberfläche. Dann produzieren sie sogenannte Adhäsine, die es ihnen ermöglichen, fester an der Oberfläche anzuhaften und nicht etwa vom Wasser abtransportiert zu werden.
