Cureus | Acute Cholelithiasis With Acute Pancreatic Calcifications: A Unique Presentation of Sickle Cell Crisis

Sickle cell disease is one of the most prevalent hemoglobinopathies around the world. Sickle cell anemia manifests due to a genetic alteration caused by substituting a single nucleotide in place of another nucleotide in the beta-globin gene. This mutation causes rigidity and sickling of the erythrocytes when the oxygen saturation falls in conditions like infection, dehydration, stress, etc. The distorted erythrocytes cause vaso-occlusion, tissue ischemia, and infarction. Cholelithiasis is known to be a frequent complication in cases of sickle cell anemia. It occurs because of chronic hemolysis. In sickle cell patients, calcific pancreatitis is an infrequent complication of vaso-occlusive crisis. It appears in connection with cholelithiasis in most patients. This case report highlights a rare presentation of sickle cell crisis complicated with multiple pigmented gall bladder calculi and pancreatic calcifications.

Cureus | A Rare Case of Neck Swelling: Acute Suppurative Thyroiditis With End-Stage Renal Disease

Acute suppurative thyroiditis is a rare life-threatening endocrine emergency. The thyroid gland has rich vascularity and lymphatic drainage, has large amounts of iodine in the tissue, generates hydrogen peroxide, and is encapsulated. Owing to these factors, infection of the thyroid gland is rare. The clinical presentation of acute suppurative thyroiditis closely resembles that of subacute thyroiditis, with a differentiation possible only on fine needle aspiration cytology (FNAC). However, differentiating these two conditions is important because the management of these two conditions differs drastically. Management includes intravenous antibiotics, drainage of abscesses, and sometimes surgery may be required. Here, we present a case of thyroid abscess caused by methicillin-resistant Staphylococcus aureus (MRSA), diagnosed using FNAC of the thyroid gland and blood culture.

Cureus | Intramedullary Spinal Tuberculoma: An Uncommon Presentation of a Common Disorder

Intramedullary tuberculoma (IMT) is rare and usually indistinguishable from spinal cord tumors. Thus, the diagnosis of an IMT is challenging. Our case deals with an unusual presentation of a 55-year-old Asian man who had presented with lower limb weakness which was found to be caused by the dissemination of tuberculosis (TB) resulting in an IMT, a rare complication of tuberculosis. The patient also had a concurrent incidental hepatitis B infection. The treatment of IMT is anti-tuberculous medication. This case highlights the significance of the prompt diagnosis of an IMT, urgent intervention particularly in developing areas of the world where tuberculosis is still endemic, an increased probability of patients having an IMT, and their diagnoses being missed.

Cureus | A Case Report of a Prevertebral Mass in an Elderly Male Post Hodgkin's Lymphoma

Immunoglobulin G4-related disease (IgG4-RD) is a fibroinflammatory autoimmune disease characterized by tissue infiltration by dense lymphoplasmacytic infiltrate composed of T cells, activated B-cells, and plasma cells expressing IgG4 and has varied presentations with similar histopathology. It can involve visceral organs, glands, aorta, lymph nodes, and retroperitoneal tissue. In our case, a 68-year-old male with a past history of Hodgkin's lymphoma and in remission presented for investigation of polyclonal gammopathy. Serum electrophoresis showed increased free kappa light chains, free lambda light chains, and kappa lambda ratio; immunoglobulin G (IgG) levels were also increased. A positron emission tomography (PET) scan and magnetic resonance imaging (MRI) thoracic spine suggested a hypermetabolic prevertebral soft tissue density. Biopsy of the mass suggested IgG4-related disease (IgG4-RD). He also had a compression fracture of the T7 vertebra. He was started on intravenous methylprednisolone and rituximab, following which he had a significant decrease in the size of the mass along with a decline in the levels of IgG, kappa, and lambda chains.
