Pandemic status: COVID 19 rages worldwide and in the United States
The pandemic is raging across the globe, is continuing to compromise health care systems regionally in the United States with lack of beds and critical resources, and is causing provider compromise/fatigue. There have been over 18 million documented cases of COVID 19 in the United States. Our current 7-day average of new cases daily in the United States is over 232,000, and we have had over 317,000 deaths.
In Virginia, the 7-day moving average of cases by date reported is 3677; there was transient improvement but case counts appear to be rising again. The 7-day average percent test positivity is currently 11.5%. Community transmission extent in all regions of Virginia is substantial (high). The moving 7-day average of people hospitalized for COVID 19, a lagging indicator, is at 2399. We still have adequate hospital and ICU capability across the state at this time, ICU occupancy is 80% and 33% of ventilators are in use. All counties across the state are at highest risk as defined by the CDC with only two counties remaining at less than higher risk.

Limitations and Safety Concerns of the Pfizer Vaccine 
The FDA's Advisory Committee on Immunization Practices (ACIP) panelists met Dec. 11 and recommended approval of the vaccine for those age 16 and older. The panel said clinicians need more guidance on whether to use the vaccine on pregnant or breastfeeding women, the immunocompromised, those who have a history of allergies and those under age 16. 
The FDA-issued healthcare provider information sheet on the vaccine said there is not enough data to recommend vaccinating those women or the immunocompromised, and also advises against giving the vaccine to individuals who have a history of serious allergic reaction to any component of the vaccine.

Pandemic status: Health care system integrity threatened nationwide
The post-Thanksgiving pandemic surge across the United States and the Commonwealth of Virginia is causing havoc within health care systems with lack of beds, resources, and provider compromise/fatigue. There have been over 16 million documented cases of COVID 19 in the United States. Our current 7-day average of new cases daily in the United States is over 220,000, and we have had over 299,000 deaths. At these high community transmission levels, the threat to our vulnerable facilities (nursing homes, prisons, and schools), our businesses, our economy, our health care system, and our loved ones is severe.

Trudeau: Canada set to receive 168K doses of Moderna vaccine by year's end


Health Canada, the country’s health regulator, is expected to approve the vaccine soon, the Associated Press reported. Deliveries could begin within 48 hours of approval.
Trudeau said that the storage of Moderna’s vaccine makes it easier to ship to remote areas, according to AP. He said the vaccine will “be directed to the north as well as remote and Indigenous territories.”

Canada has approved Pfizer and BioNTech’s coronavirus vaccine and, like the U.S., began inoculating its citizens this week, according to AP.  It is expected to receive an additional 200,000 doses of Pfizer’s vaccine as well.

FDA Clears First COVID-19 Vaccine Under an Emergency Use Authorization

The vaccine uses messenger RNA, which is extremely fragile and requires very cold storage temperatures and can be damaged by light. This is the is the first ever messenger RNA vaccine to gain FDA clearance. 
Cartons of Pfizer-BioNTech COVID-19 Vaccine Multiple Dose Vials are shipped in thermal containers with dry ice. Once received, hospitals need to remove the vial cartons immediately from the thermal container and store in an ultra-low temperature freezer between -112ºF to -76ºF (-80ºC to -60ºC). Vials must be kept frozen and have minimized exposure to room light, and avoid exposure to direct sunlight and ultraviolet light.
