Tallinn Charter 15th Anniversary Health Systems Conference: Trust and transformation – resilient and sustainable health systems for the future

On 12–13 December 2023 WHO/Europe and the Ministry of Social Affairs of Estonia will co-host a high-level health systems conference titled “Trust and transformation: resilient and sustainable health systems for the future” to commemorate 15 years of the Tallinn Charter: Health Systems for Health and Wealth.  Adopted in 2008 by all WHO European Region Member States, the Tallinn Charter recognized that health, health systems and economic development are closely intertwined, and that spending on health was an investment rather than a cost. The Charter reaffirmed countries’ support for universal health coverage (UHC) and enshrined solidarity, equity and participation as shared values which underpin European health systems. Adopted in a social and economic climate that differs greatly from today’s, the Charter addressed the needs of the time.Now, in 2023, the 15th Anniversary Conference is set against the backdrop of the legacy and lessons learned from the COVID-19 pandemic and takes place in a much-changed geopolitical, social and economic landscape.Health systems across the Region are struggling to respond to new challenges and crises. Among them are an ageing population and increased comorbidities, the uptake of digital tools in the health sector, the emergence of expensive novel medicines and technologies, the impact of climate change, and growing patient dissatisfaction – all of which seem to be pushing countries away from, rather than towards, UHC. The principles agreed during previous Tallinn health systems conferences (2008, 2013 and 2018) are enduring, and this year’s event offers an opportunity to reflect on the challenges that countries face, while looking at how they can develop resilient, responsive and sustainable health systems needed in the 21st century.Why “trust” and “transformation”  In the wake of the pandemic and the current social, political and economic climate, various studies and surveys have pointed to a growing lack of trust in institutions and politicians, with a consequent impact on health systems.People do not trust that health services will deliver for them when they are needed; health and care workers are losing trust that the system values them; and politicians do not trust health systems’ ability to reform in the face of new challenges (for example, digital innovations) or to address growing issues of concern (for example, demographic ageing and rising comorbidities).The result is that public confidence in science and health leaders has been shaken, with health professionals leaving their professions, and decision-makers concerned about system performance and unsure about where and how best to invest. With this year’s conference, WHO/Europe is working closely with the European Observatory on Health Systems and Policies to ensure an up-to-date evidence base to the conference, and is calling for a shift in the way health systems challenges are tackled, by focusing on people. People – patients, health workers, and their interactions – will be at the heart of the 2023 edition.  The first premise underlying the conference is that trust and transformation go hand in hand. The second is that transformation is more likely if people on the front line – patients and health workers – identify and tackle the problems and challenges they face.  Day one of the two-day event will focus on the need to (re-)build trust, providing concrete examples of scalable policies that reflect a sense of co-creation by patients and health professionals, while day two will address the tools and approaches needed to measure and manage transformation, highlighting examples of where, what and how to invest to support sustainable change.The 2023 Tallinn Health Systems Conference will take place in Tallinn, Estonia, and will be livestreamed on the WHO/Europe website.Breaking with the traditional approach of political and/or expert panels presenting to the audience, the conference will deliver a series of plenary sessions driven by specific questions and examples of both problems and transformations within health systems from the perspective of patients and health professionals.  A series of videos showing case studies from different countries will serve as the catalyst for discussions, with policy-makers responding to those case studies. The importance of trust and co-creation in policy design and implementation will be a common theme.The main aim of the conference is to put forward a vision for new and integrated health systems that recognize the key roles of patients and health workers, and capture their voices and needs. The conference will also:  demonstrate why reinstating and strengthening trust in our health systems is a prerequisite to transformation; highlight the importance of co-creation in policy design in order to recognize the key roles of patients and health workers, and to ensure that transformation initiatives capture their voices and needs; present an updated health system performance assessment framework that will reference important new elements, including digitalization and climate change, and taking on board lessons from the COVID-19 pandemic; and facilitate in-depth discussions on specific policy areas that are central to transformation, including: dual-track health systems; delivering in emergency contexts; digital initiatives; financial protection; engaging youth and civil society; sustainable greening of health systems; tackling workforce challenges; and precision medicine. Many high-level speakers and national dignitaries are expected to address delegates over the 2 days, and there will be parallel sessions focusing on specific examples of good practice related to transforming health systems to meet the health challenges of the 21st century.  Ultimately, the Tallinn Charter 15th Anniversary High-level Conference is expected to re-galvanize regional support for the values espoused in the 2008 Tallinn Charter to strengthen and safeguard European health systems today and in the future.
